| Cardiac Services Discovery Interviews We wish to interview adult patients and their carers (i.e. a spouse or partner, close family member or companion) who are being investigated or receiving treatment for heart disease. The purpose of the work is to hear patients and carers accounts of their experience, in their own words, by telling their story. We refer to these stories as 'discovery interviews'. This is then read out to local health care professionals involved in caring for patients with heart disease. |

| Core Strategy Place Shaping Paper Consultation upon the 'Place Shaping Paper', which considers a preferred strategy and further options for the emerging Core Strategy. It also forms the first stage of consultation upon the Hereford Area Plan and the Market Towns and Rural Areas Plan. All three documents will ultimately form part of the Local Development Framework (LDF). The Core Strategy is a long-term strategic planning document, which sets out the vision and objectives for the county and establishes the policy framework and the board locations for development necessary to deliver them. The Hereford Area Plan and the Market Towns and Rural Areas Plan will set out proposals for the delivery of sustainable growth in their respective areas. |

| Local Transport Plan 3 - Vision and Issues Herefordshire Council is asking local people and key stakeholders to help inform the development of Herefordshire's Transport Strategy for the next 15 years. People will have the opportunity to tell us what their top priorities are and which transport improvements they feel are most needed. We are particularly interested to know what residents feel about sustainable travel, using public transport and road safety. The responses to this exercise will help inform the development of the Council's Local Transport Plan (LTP) Strategy Document 2011 to 2026. The timing for the review of our Strategy also enables us to coordinate closely with proposals for the county's development over the same period. We would like everyone responding to take into account the role that transport will need to play in helping support more housing and economic development but also in helping to reduce environmental impact and climate change. |

| Living Well with Dementia in Herefordshire 1. Consultation with existing service users and their carers on how to improve dementia services in the county through the Al's Cafes and the Carer Support Group 2. Consultation with the general public on how to improve services for people with dementia in Herefordshire 3. A direct engagement exercise in the form of Dementia Postcard Campaign. Postcards would be distributed across Herefordshire to older people, people with dementia their families and carers, some by direct mail and the majority cascaded via community teams, inpatient services, GPs etc. Each postcard would ask the recipient for 'Three suggestions to improve life for people with dementia and/or their carers' The postcards would be returned using a pre-printed Freepost address and the results analysed by theme and % of respondents. |

| ContactPoint Benefits Survey Survey of all ContactPoint users trained in Herefordshire to find out how ContactPoint (and CAF) will help to increase interagency working and improve outcomes for children, young people and families. Survey taken during ContactPoint training session and repeated after six months of using ContactPoint. A similar baselining and benefits assessment process is being undertaken by all local authorities implementing ContactPoint. |
